SERVICES

  • All new patients will undergo an initial evaluation (intake) to formulate a comprehensive, individualized treatment plan. During this first appointment, we will explore your reasons for seeking treatment and discuss your psychiatric and medical history background. We will assess your health, including medications, diet and digestion, exercise, substance use, family history and relationships, occupation, and spiritual beliefs. In addition, we will review any lab tests including blood work that you may have done. When appropriate, we may also consult with other clinicians on your treatment team. Based on our thorough evaluation, we will provide you with feedback and will collaborate with you to formulate an individually tailored treatment plan to address the biological, psychological, and social aspects of your health.

    Appointment Duration: 60 Minutes

  • It is important  that all psychiatric medications that you are prescribed are managed effectively. We will monitor your symptoms and your response to medications. This helps maximize treatment effectiveness and minimize potential side effects, as well as account for changes over time.

    Disclaimer:

    prescriptions (& refills) will only be written for patients who maintain their regularly scheduled appointments and are in good standing with Integral  Behavioral Health and the prescriber. Prescriptions are written at each appointment. If you need a refill in between appointments, a prescription may be written to cover you until your next scheduled appointment, at the prescriber’s discretion. Please note it may take up to 24 hours (excluding weekends/holidays/vacations) for the prescription to be submitted after your request is received.

    Controlled Substance Prescription Laws:

    Controlled substances can only be written for 30 days at a time. This includes, but is not limited to: Adderall, Vyvanse, Focalin, Xanax, Ativan, & Ambien. The law also requires that a new prescription be written each time these medications are filled, and no exceptions can be made by your prescriber OR pharmacist for early refills or for refills >30 days. Please make sure to keep your medications locked up and in a safe place to prevent loss and theft.

  • Therapy forms a vital part of most effective mental health treatment plans.  Here at Integral behavioral health our providers  excel in several forms of talk therapy. Our providers  use brief psychotherapy techniques that produce results in a shorter time.

  • At Integral Behavioral Health, we offer personalized cutting-edge pharmacogenomic testing utilizing GeneSight labs that analyzes clinically important genetic variations in your DNA.  The results of the test can inform you and your doctor about genes that may impact how you metabolize or respond to certain medications. The genetic testing consultation service consists of two appointments. During the initial consultation, the provider will conduct a thorough assessment of your medication treatment history, go over any medication concerns, analyze your response to medications, and collect a DNA sample which will be sent to the GeneSight lab. As part of the second visit, the results from the GeneSight lab will be reviewed with a detailed explanation of your test results. A customized medication treatment plan based on your genetic information will discussed. For further information please view GeneSight lab costs.

  • Payment is due in full at the time of service , unless you are covered by one of the carriers contracted. It is advised that you contact your insurance company to confirm your coverage for services provided by Integral behavioral health services. Insurance copays, deductibles and non-covered expenses are due on the day of appointment. You will be asked to pay any outstanding balances prior to being seen unless an alternative financial arrangement has been made. By proceeding with services , you acknowledge that you are financially responsible for any remaining balance not covered by your insurance

MEET YOUR PROVIDER

Stephanie Onyeforo, DNP, PMHNP-BC, FNP-BC ,is a dedicated and board-certified Psychiatric Mental Health Practitioner and Family Nurse Practitioner at Integral Behavioral Health Services. Stephanie provides high-quality children, adolescent, and adult  psychiatry services .  Stephanie has a Bachelor of Arts in Economics and a Bachelor of Science in Nursing from the University of Nebraska Medical Center in Omaha, a Master of Science in Nursing (Family NP)  from Olivet Nazarene University in Bourbonnais, Illinois, a Post-Master's certificate in Nursing (PMHNP) from the University of Saint Francis in Joliet, Illinois, and a Doctor of Nursing Practice (DNP) from Frontier Nursing University, Versailles, Kentucky. Her clinical experience includes being a Psychiatric Mental Health Nurse Practitioner at Mathers Clinic (PMHNP-BC) and Registered Nurse at Whitehall of Deerfield Healthcare Center in Deerfield, Illinois. Stephanie is always compassionate while interacting with clients and their families, providing top-tier services to ensure patient safety and well-being. She is talented in identifying clients' needs, developing care plans based on specific needs, and adjusting as needed. She strengthens rapport with patients, families, external medical professionals, and organizations within the community. She collaborates effectively with other health care professionals and reacts composed in elevated stress situations, demonstrating confidence and sound decisions.
